Históricamente, el diagnóstico de sincope vasovagal se basó en el interrogatorio. La anamnesis sigue siendo fundamental. Desde la década de los 80 se comenzó a utilizar la prueba de ortostatismo pasivo prolongado o tilt test con fines clínicos. Al provocar un reflejo vasovagal en las personas susceptibles, se lo utiliza como método diagnóstico. Con su uso, otros cuadros clínicos como la taquicardia postural fueron identificados.
